Calvin Lee Cook of Madison, TN passed away on Wednesday, June 27, 2018, he was 62.
Survived by his wife, Yvonne B. Cook; daughter, Crystal Cook; grandchildren, Alexa Payton, Journey Cook and Dyson Cook.
A memorial service will be conducted on Monday, July 2, 2018 at 7pm in the Sanctuary of Goodlettsville Pentecostal Church 733 Old Dickerson Rd. Goodlettsville, TN 37072. Pastor Tim Zuniga and Pastor Dan Russell will officiate. Madison Funeral Home in charge of arrangements. (615) 868-9020.
